Our memory care programming is something we’re really proud of. We’re constantly working to try new approaches and engage our residents as fully as possible. We offer residents a range of cognitive exercises every day, such as current events discussions, reminiscing activities, brain teasers, and word games. Several New Haven communities have a Music and Memory program designed to immerse residents in musical interaction exercises. We’ve also incorporated other innovative activities such as our Essential Connections program—therapeutic hand massage with aromatic oils that can trigger memory. We’re currently planning to implement a specialized program for Parkinson’s patients as well.

As soon as a referral is sent over, a New Haven team member will respond within an hour to assess your patient to determine whether or not your patient is appropriate.

When you call us to inquire about a patient, we’ll send you the necessary paperwork to sign. If your patient is currently in the hospital, that includes a patient history and physical, a face sheet, a list of medications and administration instructions, and a signed admission order. If your patient is currently in another assisted living or skilled nursing facility, we can obtain documentation from that facility and will only require a new admission order.
